Veterinary Consent
In accordance with the Veterinary Surgeons Exemptions Order 2015, physiotherapy treatments given to patients with pre-exsisting condtions must have veterinary consent. Therefore prior to sessions, writen consent must be obtained from your vet. Please see the link below for the form to be filled out and then i will pass this onto the vet to receive consent. This does not come at any additonal cost and allows me to provide the best treatment for your pet.
However, for maintenance cases only, patients do not need veterinary consent (RVCS, Section 19.24, November 2020). If you have any questions regarding this or would like to check which category your pet falls into, please do not hesitate to contact me.
Payment Information:
-
Payment must be sent via BACs prior to appointment or cash at apppointment.
-
Paid invoices will be emailed alongside session reports.
-
Appologies for any inconvience caused, this is to prevent time wasted on chasing outstanding payments.
Thank you
